چکیده

We read with interest the study by Cuthbertson and colleagues [1] in a recent issue of Critical Care, and we appreciate their attempts to support studies regarding selective decontamination of the digestive tract (SDD). However, we have some concerns. The argument supporting a randomized controlled trial (RCT) for the evaluation of SDD is ambiguous. The Delphi method embraces ‘quasi-anonymity’, in which the researcher is aware of the responses of the participants [2]. The selection of experts is prone to bias, as participants were chosen on the basis of their inclusion among a particular group. Moreover, participants’ perception of SDD via a self-rated questionnaire is often misleading. Capacity to accurately reflect upon self-knowledge may be skewed, as we inflate the scope of our experience and understanding. The validity of a consensus is dubious, as dissidents are encouraged to alter their responses so as

Impact of selective decontamination of the digestive tract on carriage and infection due to Gram-negative and Gram-positive bacteria: a systematic review of randomised controlled trials.

Meta-analyses of randomised controlled trials of selective digestive decontamination have clinical outcome measures, mainly pneumonia and mortality. This meta-analysis has a microbiological endpoint and explores the impact of selective digestive decontamination on Gram-negative and Gram-positive carriage and severe infections. We searched electronic databases, Cochrane Register of Controlled Tr...

Selective Decontamination of the Gastrointestinal Tract: A Possible Strategy to Reduce Infection in High-Risk Populations

The microbial ecology in the human digestive tract is diverse and here environmental triggers, immune signals and genetic susceptibility can affect the human host’s response to infection [1]. Selective decontamination has emerged as a potential treatment for patients at high risk of infection such as those; in the critical care environments, undergoing invasive surgical operations, or with sign...
